Interoception: Understanding Body Signals Without Fear

Interoception is a quiet, constant conversation between your brain and your body. It is the sense that helps you notice hunger, thirst, temperature, pain, heartbeat, and emotion. Most of us move through the day guided by these signals without thinking about them. For many neurodivergent children, young people, and adults, and for people experiencing NEAD or other functional symptoms, interoception can feel confusing, overwhelming, or unreliable.

Understanding interoception gently and without fear is one of the most powerful ways to support emotional regulation, reduce distress, and build confidence in body‑based experiences.

What interoception actually is

Interoception is one of our eight sensory systems. Instead of noticing the outside world (like sight or sound), interoception helps us notice the inside world. It tells us:

  • My heart is beating fast

  • My stomach feels tight

  • My breathing is shallow

  • I feel warm

  • I feel shaky

  • I need the toilet

  • I’m hungry

  • I’m anxious, excited, or overwhelmed

Interoception is the foundation of self‑awareness. When it works smoothly, we can match body signals to emotions and choose helpful responses. When it’s harder to interpret, the body can feel unpredictable, and that can create fear or avoidance.

Neurodivergent differences in interoception

Many autistic and ADHD individuals experience interoception differently. This isn’t a problem or a deficit; it’s simply a variation in how the brain processes internal information.

Common differences include:

  • Low interoceptive awareness: Signals are faint, delayed, or easy to miss. A child may not notice hunger until they feel sick, or may not realise they’re anxious until they’re already overwhelmed.

  • High interoceptive sensitivity: Signals are loud, intense, or difficult to filter. A racing heart might feel frightening, or a small stomach flutter might feel like a crisis.

  • Difficulty connecting signals to meaning: “My chest feels tight; does that mean I’m scared? Excited? Unwell?” Without a clear map, the body can feel confusing.

These differences can affect emotional regulation, behaviour, and wellbeing, but with support, interoceptive awareness can grow.

NEAD and misinterpreted body signals

In NEAD (non‑epileptic attack disorder), the brain is often responding to stress, overload, or emotional intensity. Interoceptive signals can become amplified, misread, or linked to fear.

For example:

  • A racing heart might be interpreted as danger

  • Dissociation might be mistaken for collapse

  • Muscle tension might feel like loss of control

  • A wave of emotion might feel like a physical threat

When the brain misinterprets internal signals as unsafe, it can trigger protective responses - including NEAD episodes. This isn’t deliberate, and it isn’t a sign of weakness. It’s the brain trying to help, but getting the message wrong.

Building interoceptive awareness helps young people recognise early signs, understand what their body is doing, and respond with grounding rather than fear.

How to build interoceptive awareness gently

Interoception grows through slow, safe, non‑judgemental noticing. The goal isn’t to “fix” anything, it’s to help the body feel familiar rather than frightening.

Here are gentle ways to support this:

  • Start with neutral sensations: Notice breathing, temperature, or posture before exploring emotional signals.

  • Use external anchors: “My hands feel warm when I hold this mug.” Linking internal and external cues builds confidence.

  • Name sensations without meaning: “My chest feels tight” is enough. You don’t need to decide why straight away.

  • Use grounding to reduce fear: Slow breathing, sensory input, or movement helps the brain interpret signals more accurately.

  • Build a body‑signal vocabulary: Create a simple list of sensations: fluttery, heavy, buzzy, floaty, tight, warm, shaky. This helps children describe what they feel without panic.

  • Practice during calm moments: Interoception work is most effective when the nervous system feels safe.

Over time, the body becomes less mysterious and signals feel clearer.
